3. Complete the Health History, PAR-Q, and Waiver forms Download PDF.
4. Send all forms along with payment to address on the registration form.
Fee:
$169 - Extreme/Women's fitness Boot Camp (Full session/12 classes)
$15 - Per class if attending less than 12 classes.
$20 - Walk in, pay-as-you go.
